I don't remember ever hearing that Lord Gimblox did not drop a "Lord Gimblox's Signet Ring". I would say the question mark by the loot drop frequency "always?" is not necessary.
Lord Gimblox can go many hours without spawning. I've heard of a 10 hour stretch and no Gimblox, not even at the end. I've also seen Him spawn after 1-6 kills.
